Abstract

Background: Severe bacterial pneumonia can trigger complex inflammatory cascades and cytokine storms. Initiated by the activation of pattern recognition receptors (PRRs) on alveolar macrophages, the dysregulated release of key pro-inflammatory mediators—prominently IL-6 and TNF-α—shifts the immune response from a protective host defense mechanism to a driver of catastrophic tissue injury. This hyper-inflammatory state frequently leads to severe complications, including acute respiratory distress syndrome (ARDS) and multi-organ dysfunction.

Objectives: To highlight the critical role of inflammatory cascades and to evaluate the essential use of molecular biomarkers for the timely detection and clinical management of severe bacterial pneumonia.

Methods: A narrative literature review was conducted using major scientific databases, including PubMed, Scopus, and Web of Science, to synthesize current evidence on the pathogenesis of cytokine storms in bacterial pneumonia and the clinical utility of key diagnostic and prognostic biomarkers.

Results: The review delineates the diagnostic, prognostic, and therapeutic value of current molecular markers in managing hyper-inflammatory states. Additionally, it identifies transformative future directions in biomarker research, emphasizing the integration of artificial intelligence (AI), machine learning, non-invasive liquid biopsies, and multi-omics approaches.

Conclusion: A better understanding of the interplay between immunological insults and complex pro- and anti-inflammatory markers is essential. Utilizing these insights will facilitate the transition toward personalized precision medicine for critically ill patients, thereby maximizing the efficacy of immunomodulatory treatments and improving survival outcomes.
